What is the cause of infertility?

There are many possible causes of infertility, and fertility problems can affect either partner. Understanding the causes of fertility issues is the first step toward a solution.

Lack of regular ovulation: Conditions that affect the release of eggs from the ovaries. This is one of the most common causes of female infertility and can be due to hormonal imbalances, such as those caused by Polycystic Ovary Syndrome (PCOS).

Damage to fallopian tubes: Blocked or damaged fallopian tubes can prevent sperm from reaching the egg or prevent the fertilized egg from traveling to the uterus. Causes can include pelvic inflammatory disease, endometriosis, or previous surgeries.

Endometriosis: A condition where tissue similar to the lining of the uterus grows outside of it, which can interfere with the function of the ovaries, uterus, and fallopian tubes.

Uterine or cervical causes: These include abnormalities in the uterus's shape, benign polyps or fibroids in the uterine wall, or issues with the cervix that prevent sperm from traveling to the uterus.

Other causes: Age, weight, sexually transmitted infections, smoking, alcohol, environmental factors and/or stress

Males fertility causes: Factors that can impact male fertility include a low sperm count or problems with sperm quality, a history of testicular or prostate conditions, or prior treatments like chemotherapy or hernia surgery. Physical signs such as smaller-than-average testicles or swollen veins in the scrotum (varicocele) are also relevant risk factors.

Blood Tests for Female Fertility

Blood tests play a key role in evaluating female fertility by assessing hormone balance, ovarian reserve, ovulation patterns, and overall reproductive health. These tests provide insight into how well the ovaries are functioning and whether hormonal conditions may be affecting conception.

LH (Luteinizing Hormone) £78

Fertility Blood Tests

A common test carried out for patients struggling with infertility is a blood test which looks for a hormone, called progesterone, to check whether you are ovulating.

The timing of the test is based on how regular your periods are. If you have irregular periods, you'll be offered a test to measure hormones called gonadotropins, which stimulate the ovaries to produce eggs.

You can have all tests without the need for a GP NHS referral.

Endometrial biopsy (Endometriosis)

An endometrial biopsy is a simple, outpatient procedure to collect a tissue sample from the lining of the uterus. The sample is then analysed to investigate the cause of abnormal bleeding, infertility, or other gynaecological concerns. This procedure is key to diagnosing conditions such as hormonal imbalances, polyps, or pre-cancerous cells.

Hysterosalpingogram (HSG)

You will be asked to lie down on the x-ray table in a similar position to that used for a smear test. An instrument called a speculum will be inserted into your vagina (as in a smear test). A fine tube is inserted through the cervix into the neck of the uterus and held in position with a small balloon.

A small amount of colourless dye is then injected into the uterus and fallopian tubes. This dye is visible on x-ray so images will be taken at this time.

The examination normally takes 10 minutes but sometimes may take a little longer – you should allow for 30 minutes which includes time to change into your gown, the test itself, and changing afterwards.

Laparoscopy Surgery

If you're having a laparoscopy to diagnose a condition, it usually takes between 30 and 60 minutes. It'll be longer if you're having surgery to treat a condition.

The surgeon will make a small cut, usually near your belly button.
Air is pumped into your tummy through the cut so the surgeon can see inside your tummy more easily.
A thin, flexible tube with a camera inside (a laparoscope) is then put into the cut so video images of the inside of your tummy can be shown on a screen.
If you're having surgery, more small cuts may need to be made in your tummy for the surgeon to pass their tools through.
When it's finished, the air is let out of your tummy and the cuts are closed with stitches or clips, and a dressing is placed on top.

The key benefit of laparoscopy is that it is associated with less pain, a shorter hospital stay, and a faster recovery time compared to traditional open surgery.

HyCoSy Procedure

HyCoSy, stands for Hystero Contrast Sonography. This test is done to find out if your fallopian tubes are open. It can also help detect some problems with the womb (uterus) and its lining (endometrium).

Most patients have this test if they have difficulty conceiving. Having open tubes is necessary for natural conception to be achieved as it allows the sperm and egg to meet.

The procedure is carried out in the outpatient department. An internal (transvaginal) ultrasound is carried out first. A medical instrument called a speculum is then placed in the vagina while you are awake and lying on your back, and a thin plastic catheter is passed through the neck of the womb. A further internal ultrasound is then carried out and a dye is flushed through the catheter at the same time. The procedure itself lasts about 20 minutes.

What are the risks?

The procedure is considered safe, well tolerated and rarely causes any complications.
Pelvic infection: 0-3%; if you develop an offensive vaginal discharge or high temperature, please contact our medical team as you may require antibiotics.

Inconclusive procedure: due to discomfort or technical difficulties.

Perforation of the womb: less than one in a 1,000

Allergic reaction to the dye: less than one in 100

Do I need a GP referral to see a fertility specialist?

Are you more fertile after a miscarriage?

It’s possible to be more fertile in the first few weeks after a miscarriage, especially if your menstrual cycle returns quickly. Hormone levels and ovulation can normalize within a couple of weeks, and some women do conceive soon after. However, every body is different, so it’s important to speak with a healthcare provider about when it’s safe and appropriate to try again.

Generally, the least fertile phase is outside of the ovulation window, which typically occurs mid-cycle (around day 14 in a 28-day cycle).
