Rhinoplasty: Your Most Common Questions Answered by a Consultant Plastic Surgeon

What is rhinoplasty?

Rhinoplasty is surgery to change the shape, structure, or function of the nose. It can be performed for cosmetic reasons, breathing problems, or both.

Some patients want to refine the appearance of their nose, while others need surgery to correct a deviated septum or nasal obstruction that affects breathing. In many cases, cosmetic and functional improvements can be achieved during the same procedure.

What’s the difference between cosmetic and functional rhinoplasty?

Cosmetic rhinoplasty focuses on the appearance of the nose, such as:

  • Reducing a nasal hump

  • Refining the nasal tip

  • Improving facial balance

Functional rhinoplasty focuses on breathing and airflow, often correcting issues like a deviated septum or collapsed nasal valves.

Many patients require a combined approach, where both appearance and breathing are addressed together.

A beautiful nose must be a functional nose too.

Can rhinoplasty help with breathing problems?

Yes. Rhinoplasty can significantly improve breathing when nasal obstruction is caused by structural issues.

If you experience:

  • Difficulty breathing through your nose

  • Chronic nasal blockage

  • Snoring related to nasal airflow

You may benefit from functional rhinoplasty or septorhinoplasty, which aims to restore normal airflow while maintaining a natural appearance.

How do I know if I need septorhinoplasty?

Septorhinoplasty is usually recommended when both the shape of the nose and the nasal septum need correction.

If you have a visibly crooked nose, breathing problems, or a history of nasal injury, septorhinoplasty may be the most effective option. This will be discussed in detail during your consultation after a thorough assessment.

Is rhinoplasty painful?

Rhinoplasty is not usually described as painful, but it can feel uncomfortable during the early recovery period.

What do patients often find uncomfortable, and how do we minimize this?

  • Nasal packing: Many patients find nasal packing particularly uncomfortable. We do not routinely pack the nose, allowing you to breathe through your nose immediately after surgery.
  • Stitch removal: Stitch removal can be unpleasant due to a pulling sensation. We use specialist scissors designed to cut sutures cleanly without pulling, making the process more comfortable.
  • Sore throat: Some patients report that a sore throat after surgery can be more uncomfortable than nasal soreness. This is usually caused by a throat pack used during the operation. We no longer use throat packs, significantly reducing postoperative throat discomfort.

Pain is usually well controlled with medication, and discomfort improves steadily over the first week.

How long does rhinoplasty recovery take?

Initial recovery typically takes 7–10 days, during which swelling and bruising gradually settle.

  • Most patients return to work within 1-2 weeks

  • Light activities can resume after 2 weeks

  • Gentle cardio exercise like a gentle walk after 2 weeks

  • After 4 weeks you can increase the intensity somewhat

  • After 6 weeks you can go back to strenuous activity like weight lifting

  • We recommend 3 months before returning to contact sports

  • Full healing continues over several months

While you’ll notice improvements early on, the final shape of the nose can take up to 12 months to fully settle. In some cases it can take longer depending on many factors.

When can I return to work after nose surgery?

This depends on your job and comfort level.

Many patients return to non-physical work after 10–14 days. If your role involves physical activity, you may need more time off. This will be discussed during your consultation so you can plan appropriately.

How long do rhinoplasty results last?

Rhinoplasty results are long-lasting and usually permanent.

Once healing is complete, the changes made to the structure of the nose are designed to remain stable over time. Maintaining good nasal care and following post-operative advice helps ensure the best long-term outcome.

Your face will continue ageing and as such your nose will also age. Similar to how your features look different in your 20s compared to your 40s compared to your 60s.

Are there risks with rhinoplasty?

As with any surgery, rhinoplasty carries some risks, such as:

  • Bleeding or infection

  • Swelling lasting longer than expected

  • Changes in sensation particularly numbness of the front teeth and tip of the nose, however this usually improves within 6 months

  • Breathing issues requiring further treatment

  • A hole in the septum (middle partition) - this is very rare.

  • Scarring between the sidewall and septum

Choosing an experienced consultant surgeon and attending follow-up appointments greatly reduces these risks.

How is male rhinoplasty different from female rhinoplasty?

Male rhinoplasty usually aims to preserve a strong, natural nasal structure, while maintaining facial balance.

The goals and techniques differ slightly and careful planning is essential to avoid an over-refined appearance. Each nose is assessed individually, regardless of gender.

What makes a good rhinoplasty surgeon?

A good rhinoplasty surgeon should:

  • Be fully trained and experienced in both cosmetic and functional nasal surgery

  • Feature on the GMC Specialist Register for Plastic Surgery, Otolaryngology (ENT) or Oral and maxillofacial surgery.

  • Take time to understand your concerns

  • Explain realistic outcomes clearly

  • Focus on natural, balanced results

Rhinoplasty is one of the most technically demanding cosmetic procedures, so experience and careful planning are key.

What happens during a rhinoplasty consultation?

During your consultation, you can expect:

  • A full assessment of your nose and breathing

  • Discussion of your goals and concerns

  • An explanation of suitable surgical options

  • Simulated images of potential outcomes

  • Honest advice on expected outcomes and recovery

This is your opportunity to ask questions and ensure you feel comfortable moving forward.

Will I need cartilage taking from my ear or rib?

In most cases the cartilage of the septum is enough to do the surgery. If there has been significant injury or multiple injuries in the past then we will talk about taking cartilage from the ear or rib.  The other reason for cartilage harvest is revision surgery where previous surgery has been done and there is unlikely to be much cartilage that we can use.

Ear cartilage is fairly flexible and is useful for grafts that do not bear much of a load. However, if your septum needs rebuilding, then rib cartilage is necessary. Think of the septum as the foundation of the nose. Without a strong foundation, the structure built on it will collapse. Rib cartilage is strong and resilient.

The need for these materials are decided on a case by case basis and will be discussed at your consultation.
